Twice a year, a licensed geriatric pharmacist conducts a comprehensive review of each resident’s medications. Rather than looking at medications one by one, the pharmacist considers the full clinical picture.

What the Pharmacist Evaluates

During a Drug Regimen Review, the pharmacist carefully examines:

  1. Medical history, diagnoses, lab results, and vital signs
  2. All prescriptions, over-the-counter medications, vitamins, and supplements
  3. Whether each medication has a clear clinical purpose
  4. Potential duplicate, outdated, or unnecessary medications
  5. Effectiveness, side effects, and possible drug interactions
