Case Presentation

Resident moved into Morning Pointe on 10/16/25. At this time resident was failure to thrive with no social interaction, poor nutritional intake and underweight. Upon evaulation by the on-site physical therapist partner at Morning Pointe, the resident had the following scores: a gait goal at 20 feet and Timed Up and Go (TUG) of 120 seconds. Resident was weak and needed assistance with activities of daily living (ADL). 

The Approach

The resident’s individual plan of care focused on consistency and encouragement including support of ADL from resident assistants, on-site therapy, nutritious meals, and daily social engagement. 

 The Progress

Through consistent in-house therapy and structured routines, the resident’s strength and mobility gradually improved. Confidence grew along with improved social engagement, and the resident gained weight with 3 meals a day. 


Life Today

Status as of February 2026 – resident continues to enjoy social interactions, regular meals and scheduled activities. Resident showed a 14% gain and is maintaining a healthy weight. Test scores have greatly improved. Gait goal has increased to 75 feet. TUG is now completed in 112 seconds.
