Twice a year, a licensed geriatric pharmacist conducts a comprehensive review of each resident’s medications. This review ensures that every prescription remains appropriate, effective, and aligned with the resident’s current health needs. Rather than looking at medications one by one, the pharmacist considers the full clinical picture.

What the Pharmacist Evaluates

During a Drug Regimen Review, the pharmacist carefully examines:

  1. Medical history, diagnoses, lab results, and vital signs
  2. All prescriptions, over-the-counter medications, vitamins, and supplements
  3. Whether each medication has a clear clinical purpose
  4. Potential duplicate, outdated, or unnecessary medications
